JOB PURPOSE:
The Medical Discharge Liaison continuously monitors cases to be medically cleared. This position is responsible for the medical clearance of charts designated for discharge. The Medical Discharge Liaison monitors medical clearance lists, emails, and special cases as requested by Case Management with a focus on ensuring efficient reunification of UCs in a medically responsible way. This position will serve as the Medical Operations POC for Case Management. This role performs daily inspections, and investigations to identify and mitigate conditions which would result in errors and inconsistencies within the department. This position ensures that corrective actions are taken to ensure compliance with applicable statutes, regulations and contractual obligations. Responsible for making sure all questions regarding medical records, referrals, and medical discharge at the Pecos Children's Center have been addressed.
