JOB PURPOSE:
The case manager is responsible for using sound professional judgement and best practices to ensure quality case management services are provided to the service population especially in crisis situations while maintaining compliance with established states and federal standards. Monitors assigned caseload of clients participating in a short-term life management program; establishes program plan/goals and evaluates client’s progress by conducting routine meetings with clients and/or staff; serves as advocate for clients in order to acquire services that will enable them to functionally cope with their environment.
